Welcome to Moonbridge Therapy (“we,” “our,” or “us”). These Terms and Conditions (“Agreement”) govern your use of our services and website. By accessing or engaging with Moonbridge Therapy, you agree to comply with these terms.
Services provided
Moonbridge Therapy offers counseling and therapeutic services, including but not limited to:
- Individual therapy
- Couples therapy
- Family therapy
- Group therapy
All services are provided by licensed professionals and are tailored to meet the unique needs of each client.
Appointments and cancellations
- Scheduling: Appointments can be scheduled via phone, email, or our online booking platform.
- Cancellation policy: Clients must provide at least 24 hours’ notice to cancel or reschedule an appointment. Cancellations made with less than 24 hours’ notice may incur a cancellation fee equivalent to the session cost.
- No-show policy: Failure to attend a scheduled session without prior notice may result in a full session charge.
Payment terms
- Payment: Payment is due at the time of service unless prior arrangements have been made.
- Insurance: Clients are responsible for verifying insurance coverage and paying any co-pays, deductibles, or uncovered amounts.
- Outstanding balances: Unpaid balances must be settled before scheduling future appointments.
Confidentiality
- Client privacy: All communications and records are confidential and protected under HIPAA regulations.
- Exceptions: Confidentiality may be breached where there is a legal obligation, such as reporting abuse or neglect, preventing harm to self or others, or a court order requiring disclosure.
Client responsibilities
- Clients are expected to actively participate in sessions and adhere to the therapeutic process.
- Clients must inform therapists of any changes to their contact information, health status, or insurance coverage.
Therapist responsibilities
- Moonbridge Therapy will provide a safe, supportive, and non-judgmental environment.
- Our therapists will adhere to ethical standards and maintain professional boundaries.
Limitations of service
- Therapy is a collaborative process, and outcomes are not guaranteed.
- Moonbridge Therapy does not provide emergency or crisis services. In case of an emergency, clients should contact 911 or their local crisis hotline.
Termination of services
- Either party may terminate the therapeutic relationship at any time.
- Moonbridge Therapy reserves the right to terminate services if a client violates these terms or exhibits behavior that threatens the safety of the therapist or other clients.
Use of website
- Content: The content provided on our website is for informational purposes only and does not constitute medical or therapeutic advice.
- Third-party links: Our website may contain links to third-party sites. We are not responsible for their content or privacy practices. See our Privacy Policy.
Liability disclaimer
Moonbridge Therapy is not liable for any direct, indirect, or consequential damages resulting from the use of our services or website. Clients agree to release and hold harmless Moonbridge Therapy and its staff from any claims arising from their engagement with our services.
Governing law
These Terms and Conditions shall be governed by the laws of the State of Utah.
